چکیده
Dr. Marcus Sabry Azar Batista – Rua Olavo Bilac 1737 64001-020 Teresina PI Brasil. E-mail: [email protected] Severe hypoglycemia may occur as a complication of insulin therapy or as a consequence of accidental or suicidal insulin injections. It may result in a variety of neurological symptoms such as focal deficits, epileptic crisis, coma state and death. There is a relative rarity of reports about hypoglycemic-induced brain lesions. In newborns, the parieto-occipital region is the most affected. In adults, there is a greater variability, with reports of lesions in basal ganglia, hippocampus, temporal and occipital gray matter, insula cortex, corona radiata and splenium of the corpus callosum. The aim of this study is to report the case of a patient presenting coma, with left hemiparesis and brain lesions detected by computerized tomography scan (CT) and conventional magnetic resonance imaging (MRI). He had insulin-dependent diabetes mellitus and had a record of severe hypoglycemic episode.

Reversible reduction of apparent diffusion coefficient values in bilateral internal capsules in transient hypoglycemia-induced hemiparesis.
A woman aged 68 years who experienced recurrent right hemiparesis caused by hypoglycemia was admitted to our hospital. When she was experiencing a low level of glucose, diffusion-weighted MR imaging showed the presence of hyperintensity lesions in the bilateral internal capsule. Diffusion-weighted MR imaging has been infrequently performed in patients with hypoglycemia. We report the reversible...
متن کاملInternal capsule and splenial lesions in hypoglycemic hemiparesis.
A 41-year-old man with a 7-year history of insulindependent type I diabetes mellitus complained of right hemiparesis and was referred to our stroke center. Neurological examination revealed that his consciousness was alert, and he had dysarthria and right hemiparesis. Laboratory test values were within normal limits except for plasma glucose level (46 mg/dL). The dysarthria and right hemiparesi...
متن کاملSevere Hypoglycemia-induced Right Hemiparesis with Reversible Diffusion Restriction in the Left Internal Capsule Due to Combination Therapy Using Disopyramide and Clarithromycin
Severe hypoglycemia is known to cause acute focal neurological symptoms. In cases with a medical history of diabetes mellitus (DM), the diagnosis and treatment of hypoglycemia-induced neurological symptoms are simple. However, severe hypoglycemia can occur in patients who are not taking hypoglycemic agents such as insulin or long-acting sulfonylurea drugs. We describe a 95-year-old man with sud...
متن کاملThe Health Service and Economic Impact of Glucagon Rescue Administration During Severe Hypoglycemic Events.
■ ■ Hypoglycemia can be mild or severe. Mild hypoglycemia is usually transient and self-treatable with glucose or other sources of carbohydrates.2 Severe hypoglycemia is characterized by coma or convulsions and can precipitate hemiparesis or stroke, and can be fatal.3 Treatment by a caregiver or health professional is required, and typically involves the administration of glucagon to increase b...
متن کاملBrain lesions in the course of generalised tetanus.
A 47-year-old woman developed left hemiparesis primarily affecting the lower limbs during the course of severe generalised tetanus. MRI on the 82nd hospital day revealed cortical and subcortical lesions predominantly in the right frontal and parietal lobes in addition to marked brain atrophy. Three months later, the enhancing lesions were still present on follow up MRI. We postulate that struct...
